If you can't get her to spawn when you loot the quintessence then try this (worked for me) - rest and reload, go back to the area and kill everyone. Transition into Umbral BEFORE you pass the gate (don't just lift your lamp) and walk all the way up to where the quintessence was. She spawned for me the second time when I followed these steps

She's just bugged. She sometimes spawns under the floor which is why you can't see her.

Sometimes she will spawn mid through the floor and that's the worst that can happen because you can kill but the drop will be under the floor (you can see a bit of the orange tail) but you can't pick it, and moving zones or resting doesn't fix it.

Stand in the area where the saintly quintessence is for a few seconds for her to spawn. I killed the sister and the archer first and stood on the tower for a few seconds before she spawned

I found where this item actually is. There are 3 Abbess in the Abbey of Hallowed Sisters, one being the first boss, the second being situated just before the Rapturous Huntress of the Dusk boss room, and the third being the titular Abbess we need to kill for the Tattered Banner. This Abbess is found by going into the boss room, don't interact with the item that starts the boss instead walk forward to the middle of the room and turn to the left there you will find a flight of stairs that leads up. Take those stairs up and once at the take the hallway on the right. At the end is a door that leads to an outdoor area with several enemies including dogs. Is this actually accurate? I just killed the abbess shooting the laser beams at me in the small tower and she didn’t drop this, only a large mana stone, and she does not respawn.
